Kentucky In-School Transition Survey (Student Survey)
Dear Student,
This survey asks questions about your experiences in high school and your plans for the future. Although you are not required to complete the survey, any information you provide will be used to improve high school programs that prepare students for adult life.
You should mark only one response item to each question unless you are instructed to mark all that apply. If your answer is not listed, mark 'other' and write your answer on the line provided. If you do not understand something, stop and ask for help.
Please note, once you begin the survey, you must complete it. If you leave the survey prior to completion, you will lose all information entered and have to start over!
Your privacy is important and none of your responses can be linked back to you personally. Your name will not be on the survey and results will be collected and analyzed by staff at the Kentucky Post School Outcomes Center. No reports will include any identifying information.
